CoreCare Solutions is a proud participating provider in the Georgia Pediatric Program (GAPP) — a Medicaid-funded program offering free in-home nursing and personal care services to medically fragile children under 21 years of age.

To qualify, children must require skilled nursing care due to complex medical conditions affecting multiple body systems. Eligibility is based on a medical necessity determination by the state.

GAPP In-Home Nursing Services Include:

• Medication administration & monitoring
• Vital sign checks (BP, pulse, respiration, temperature)
• IV therapy and wound care
• Tracheostomy and ventilator support
• Gastrostomy (G-tube) feedings
• Management of gastrointestinal disorders
• Assistance with daily living activities (bathing, grooming, feeding)
• Mobility and transfer assistance
• Light housekeeping related to patient care
